Test Requisition

The T-SPOT®.TB test requisition form must be completely filled out in order for the specimen to be processed.

NOTE: Institutions may use their own test requisition form. Please ensure that it is indicated on the form that the test being ordered is "T-SPOT.TB test" and that the requisition form is completely filled out.

The T-SPOT.TB requisition is a multi-part form. The white copy should accompany each blood specimen.

NOTE: When sending multiple blood specimens from different individuals, separate requisition forms must accompany each blood specimen.
