Shipping Instructions

1. Seal box with packaging tape.

2. Place the enclosed FedEx Billable Stamp on the top of the package.

3. If your location does not have a regularly scheduled FedEx pickup, call 1-800-463-3339 to request one. Press "0" and specify that you are shipping with a billable stamp.

NOTE: The Oxford Diagnostic Laboratories shipping container is labeled and meets the requirements for Biological Samples Category B. Federal Express (FedEx) shipping regulations require that diagnostic shipments be packaged in accordance with IATA Packaging Instructions 650.

It is the shipper's responsibility to ensure that the shipping container conforms to FedEx guidelines along with all applicable local, state, and federal regulations.
