Order Supplies

There are two options for ordering supplies:

The supplies that can be ordered are:

• Oxford Diagnostic Laboratories Test Requisition Forms

• Shipping Containers and Packaging Material (Cryopak^®s)
   Oxford Diagnostic Laboratories will track each customer's usage of
   shipping containers and resupply as needed.

• Draw Station Bulletin (Collection Instructions)

• Lab Bulletin (Collection, Packaging and Shipping Instructions)

• Customer Brochure (Information for healthcare workers, students, patients) Available in: English, Spanish, Chinese, Korean, Arabic, and Bosnian.

Orders received by 2:00PM EST will be shipped the same day.  Orders received after 2:00PM will be shipped the next day.
