1. Contact Oxford Diagnostic Laboratories at 1-877-59 TBLAB A member of our Client Services Team or a Territory Manager will assist you in setting up an account.
2. Collect/Label the Blood Specimen(s)
• Blood specimens are collectedin a standard lithium or sodium heparin tube
• Label the tube with the following information
• Patient Name
• Patient Identifier (e.g. Medical Record Number)
• Patient Date of Birth
• Date and Time of Collection
• Complete the Test Requisition
3. Send the Blood Specimen(s) to Oxford Diagnostic Laboratories
• Packaging materials and shipping containers are provided
• These containers have been validated to ensure that the blood specimens are maintained at room temperature during transit
• Package the blood specimen(s) in the shipping container
• Specimens will be picked up by FedEx and shipped directly to Oxford Diagnostic Laboratories
